I am an ARDS survivor with permanent lung damage resulting in loss of 40% lung functionality. Would I be considered under CDC guidelines as extremely vulnerable to Covid-19?

I think so.  This CDC web page provides some guidance. One of the conditions they suggest for high risk of severe disease is pulmonary fibrosis. Also cystic fibrosis and COPD. They do not specifically address ARDS survivor with severely reduced lung function, but I think this is consistent with the theme of severe chronic lung disease. I think a physician may need to support this with a letter saying what the condition is, how severe it is (supported perhaps with pulm function testing).  Please see the CDC website on vulnerable populations:
